Proof of Vaccination at Bluewater Gymnastics Club

On September 15, 2021, further clarification for the Proof of Vaccination for Recreation Facilities was provided to Gymnastics Ontario and Lambton Public Health by the Government of Ontario.

Effective September 22, 2021, patrons must show identification and proof of being fully vaccinated against COVID-19 before they can enter the facility.

• Parents attending classes with children in Gymtots, Kindergym, Shooting Stars and Mini-Stars,
• Patrons who stay to watch their child in a different program

Should an individual choose not to share the required information then, unless they are exempt from the requirements, will not be permitted to enter the facility.

In individual is considered fully vaccinated if they have received:

• A full series of COVID-19 vaccine authorized by Health Canada, or any combination of such vaccines, or
• One or two doses of a COVID-19 vaccine not authorized by Health Canada followed by one dose of a COVID-19 mRNA vaccine authorized by Health Canada, or
• Three doses of a COVID-19 vaccine not authorized by Health Canada and
• They received their final doze of the COVID-19 vaccine at least 14 days before providing the proof of being fully vaccinated.

Exemption

The following patrons are exempt from the Proof of Vaccination requirement:

• Who are under 12 years of age,
• Who are under 18 years of age, and who are entering the indoor premises of a facility used for sports and recreation fitness activities solely for the purpose of actively participating in an organized sport.

Proof of Vaccination does not apply where a patron entering an indoor area solely.

• To use a washroom
• To make a retail purchase

Proof of Vaccination procedures starting Wednesday, September 22, 2021

1. A table will be set up at the entrance of the facility.
2. A designated person will be assigned to review the identification and proof of vaccination form
3. Individual’s name will be entered beside your child’s name and a “V” will be entered into the column beside your name. We will not copy or keep any of your information. 
4. Parents/Guardians who cannot provide identification and proof of vaccination will not be permitted to enter the facility.
5. This information will be required on the first visit starting September 22, 2021. A master list will be maintained for the duration of this requirement.
